Greetings!

I wish to inform you that our Society is compliant with the Data Privacy Act of 2012. In our Organization, the Board of Trustees is the Personal Information Controller (PIC) who controls the collection, holding, processing or use of personal information, including a person or organization who instructs another person or organization to collect, hold, process, use, transfer or disclose personal information on his or her behalf. Our Data Protection Officer (DPO) is Dr. Aileen P. Tomboc, she monitors and ensures our Society’s compliance with the DPA; its IRR; issuances by the NPC and other applicable laws and policies. Data Privacy Policy was drafted to inform you of our data protection and security measures, and may serve as your guide in exercising your rights under the DPA.

In order for us to fully implement our data privacy policy, all members should update their records and sign the Consent Agreement.

Step 3: Save the accomplished form as a pdf file.
Use “MDR” and your name as the File name
(i.e. MDR-JUAN DE LA CRUZ, MD)
